Kindergarten Registration for 2012-2013 School Year

* Your child must be five years of age by August 31, 2012 to register.
Please bring the following items for registration:
A certified copy of your child's birth certificate
An immunization record for your child
Proof of residence - recent utility bill (gas, water, or electric)
Picture ID for parent or legal guardian

A Kindergarten Health Assessment Form will be given to you at registration. This form should
be completed by your child's health care provider & returned on or before the 1st day of school.
